Creating a Care Plan

A Care Plan lists the medications that are to be monitored through Continga vDOT. Each medication added to the care plan details the dose and frequency. The patient, once logged in, can select one or more medications to take from their Care Plan.
A patient's Care Plan can be accessed by first selecting a patient, and then selecting the Care Plan
tab.
Within a selected patient's Care Plan, click the + New Item
button. This will open a form.
Select the medication to add to the Care Plan. This dropdown can be scrolled, or typed in with autocompleted medication options to select from.
Input the appropriate and correct Dose.
Define the Frequency and time periods that the medication should be taken, and click + Add
. Note that multiple frequency day and times can be added.
When complete, click Save
to add this medication to the patient's Care Plan.
Care Plan Items can be edited, allowing for adjusting of the dose, or frequencies / time. To do so, simply click on the item to edit from the patient's Care Plan.
Care Plan Items can be archived should the patient no longer be taking this item. This will still appear in the Patient Summary report, but will not show as a medication to take to the Patient. To do so, simply click on the item to archive from the patient's Care Plan, and click archive.
Note that the medication list can be updated. If medications you require are missing, please contact support@continga.co.uk.
